There is a new General Aviation Report form now in use, it has some changes. Can be downloaded HERE

Yes and another good thing about it is it can be email'd instead of faxed

The link Jon gave was to a PDF file so how would you fill in and email this ?
The previous version was available on the Herts Police website in both PDF and Word format so you could fill in the Word form on the screen and send it by email. Is there a 'screen fillable' version of this revised form ?
