AvMap Ultra EFIS MOD 7 question

Hello!

I am fitting a AvMap Ultra EFIS to my aircraft. Its not replacing any primary instruments its purely to fill a hole and to give me an artificial horizon and second compass. What forms am I required to fill in for this? I am filling in a mod 7 as we speak due to a trig upgrade (8.33) do I just add it on there and leave the EASA/CAA/FAA approval section blank? I have tried to find any sort of approval for this device but failed but have seen that quite a few LAA members and there aircraft have this device fitted.

Any guidance would be appreciated
